Twenty year old Jacob has been diagnosed with a life limiting condition called DiGeorge Syndrome since birth that has had a severe impact on his mental development, physical development and ultimately his life expectancy.
Despite the challenges that Jacob faces, his personality and courage shines through everyday and he is truly an inspirational character who touches the hearts of everyone he meets.
Jacob is a West Ham Utd fan and his dream was for a truly special experience alongside West Ham Legends.
The Dream Factory arranged for Jacob and his family to attend a very special Legends Game at Dagenham & Redbridge Football Club where Jacob was treated like a VIP. Jacob travelled in style to watch the match, had a very special lunch and he was also able to go down to the pitch and was presented with some very special gifts.
